About The Position

Imagine360 is seeking a Supervisor, InterDisciplinary Consult Team (IDCT) to join the team! The Supervisor, InterDisciplinary Consult Team (IDCT) provides leadership and supervision to a diverse team of clinical professionals, ensuring high-quality interdisciplinary consults and care coordination for members under group health plans. This role leverages clinical expertise, supervisory experience, and collaborative skills to support consults across nursing, behavioral health, nutrition, and care management disciplines. The Supervisor is a champion for innovation, transformation, and scaling of consult services, driving strong patient outcomes and fostering a culture of empowerment and retention. Position Location: 100% remote

Requirements

  • Nursing degree from an accredited college, university, or school of nursing.
  • 3 years' experience in a clinical role with responsibilities for direct patient care.
  • Experience in Case Management or transferable clinical experience and skills.
  • Ability to work independently in a home office environment.
  • Proficiency in Microsoft Outlook, Word, Excel, PowerPoint, and internet navigation.
  • Ability to resolve problems independently and multi-task effectively.
  • Strong written, oral, and telephonic communication skills.
  • Strong presentation and facilitation skills.
  • Commitment to building new skills and fostering a positive work environment.
  • Demonstrated organizational, analytical, and problem-solving skills; detail-oriented.
  • Ability to prioritize workloads and manage deadlines.
  • Ability to maintain confidentiality of protected health information in compliance with HIPAA.
  • An active and unrestricted Registered Nurse License in eNLC compact state. Must maintain CEUs as required by applicable State Board(s) of Nursing and required certifications. Must be willing to obtain and maintain additional license as required to perform the job functions of the organization.
  • Must be a Certified Case Manager or eligible to sit for the Certified Case Management Exam within 3 years of starting employment with i360.

Nice To Haves

  • Bachelor's degree in nursing preferred but not required.
  • Experience mentoring or managing a team of clinical or non-clinical staff.
  • Experience working in a URAC accredited Case Management and/or Disease Management program preferred.
  • Experience working within an insurance agency or TPA experience specializing in employee benefits and self-funded, ERISA medical plans.

Responsibilities

  • Edit, maintain, and implement Policies & Procedures that meet applicable accreditations and business needs.
  • Provide oversight and education to team members on topics related to interdisciplinary consults, case management, and reviews of medical necessity.
  • Review and audit monthly billing for consults and team activities.
  • Complete duties to support Quality Program initiatives, including collection of data, reporting, and training of employees to meet quality program goals.
  • Maintain and report results of satisfaction surveys.
  • Research member complaints and provide resolution.
  • Participate in interview processes and onboarding of new employees.
  • Establish orientation schedules, coordinate, and participate in new employee training as directed.
  • Oversee and evaluate caseloads for applicable employees based on eligibility criteria, member needs, and employee qualifications.
  • Monitor employee schedules and PTO coverage to meet business needs.
  • Responsible for day-to-day management and performance of the team; provide mentoring and coaching as needed.
  • Lead projects that promote improvement in delivery of services.
  • Complete and report quality assurance reviews, call and chart audits of team members.
  • Lead innovation and transformative initiatives to continuously improve consult processes, patient outcomes, and team effectiveness.
  • Develop and implement scalable consult models and forecast resource needs to support organizational growth and evolving member populations.
  • Foster a culture of empowerment, engagement, and professional growth, focusing on strong retention and positive leadership.
  • Mentor and coach team members to maximize their potential and job satisfaction.
  • Champion advanced analytics and real-time data tracking to drive proactive, measurable improvements in patient outcomes.
  • Oversee the collection, analysis, and presentation of consult-related data, including patient outcomes, team performance, and operational metrics.
  • Identify trends, share insights, and recommend actions to leadership and stakeholders.
  • Facilitate effective collaboration among RNs, LPCs, Social Workers, Registered Dieticians, UM/BR Coordinators, and Behavioral Health Support to deliver comprehensive consults.
  • Lead, attend, and participate in 1:1s, team meetings, trainings, and other job-specific events as required.
  • Communicate (in compliance with HIPAA) with brokers, vendors, Relationship Managers, HR representatives, and stop loss partners as needed.
  • Communicate professionally and effectively with internal and external stakeholders.
  • Staff complex cases with Manager, Case Management or Senior Manager, Patient Care Services.
  • Adhere to established internal regulations regarding Department of Labor, HIPAA, ERISA, and department and company policies and procedures.
  • Complete HIPAA training annually.
  • Perform all tasks in accordance with HIPAA/PHI guidelines.
  • Appropriately escalate difficult issues.
  • Complete duties in accordance with scope of licensure and certifications held or requested.
  • Perform other duties and projects as assigned.
