This is a professional position providing supervisory oversight for Family Works Program – Homeless Services under the Housing First Division. The Family Works – Homeless Services program provides an intensive wraparound approach to working with children from identified targeted schools in Pinellas County and their families to decrease homelessness and achieve stable and permanent housing potential. Family Works – Homeless Services utilizes the Housing First model and the five core principles of: immediate access to permanency housing with no housing readiness requirements; individual’s choice and self-determination; recovery orientation; individualized and client-driven supports and social and community integration. The Family Works team insure that individuals and the family are assessed and linked, to mental health, social, and educational aspects. The Family Works – Homeless Services team will collaborate and develop service plans in partnership with the family and work closely with the family to identify services needed in the home to maintain a stable environment. Services are provided in-home or in the community. The program works closely with the family to connect with resources and offer educational, financial or other assistance as needed or appropriate. The Family Works – Homeless Services Supervisor will play an integral role of support, coaching and guidance for a team of Behavioral Health Case Managers serving youth and families who are living homeless, doubled up or are at risk of homelessness. This position is assigned to oversee Behavioral Health Case Managers working with youth and families with housing needs in addition to a substance abuse, mental health or co-occurring mental health & substance abuse concern, and is responsible for assist in guiding the Behavioral Health Case Managers that are serving as a system navigator who supports the youth and family in achieving stable housing and treatment/recovery. The supervisor will ensure that the Behavioral Health Case Management services being delivered are individualized, recovery-focused and allow individuals the opportunity to learn how to manage their own recovery and advocacy process. The supervisor will be a subject matter expert on interventions of Behavioral Health Case Managers that serve to enhance the development of natural supports, as well as coping and self-management skills and for navigating homeless services/ funding within Pinellas County. The Family Works – Homeless Services Supervisor will ensure that Behavioral Health Case Managers advocate for the youth and family, link the youth or family with needed services, provide education on mental health and/or substance abuse, monitor progress and assist in the development of individualized supports, and encourage the development of skills necessary to live stably in the community. Family Works – Homeless Services Supervisor will ensure services are provided in community locations which may include the family home, medical and behavioral health sites, hospitals, community resource sites, and shelters. The supervisor will ensure that Behavioral Health Case Managers provides recommendations and referrals to other service providers as appropriate, such as therapy and/or a psychiatry, and possibly SOAR, and that the Behavioral Health Case Managers collaborate with the other clinical service providers. Family Works – Homeless Services Supervisor will also ensure that all persons served by any member of the Family Works team who are uninsured are enrolled in the Pinellas Health Plan. The Family Works – Homeless Services Supervisor is responsible for ensuring an annual client facing client care productivity number of 9,984 hours which reflects a minimum of 1,248 hours of client facing client care per Team Member on the team. Achieving the minimum required weekly, monthly and annual client facing client care hours is an essential function of this position and a performance indicator. Directions for Living is an essential services provider. All Directions for Living Team Members must be willing and able to provide vital support and essential services to the people that we serve in the event of an emergency, pandemic or other disaster situation.
