About The Position

We're the Community Healthcare Access Team (CHAT), and we partner with many community organizations to help low income and uninsured residents of EvergreenHealth's service district. Our programs are supported by tax levy funds and serve residents living in cities within King County Public Hospital District No. 2. One of the team's primary roles is to assess, coordinate and make appropriate referrals for your care within and outside the EvergreenHealth network.
