More about our team The Care Navigator team is an integral part of the Clinically Integrated Network (CIN) team, providing direct patient support to patients at-tributed to the Clinically Integrated Network by serving as a patient advocate and navigator. The CN team is responsible for assessing patient needs and assisting with the coordination of care across settings in accordance with population health initiatives. The CN team plays a key role in promoting healthy behaviors and improving patient outcomes through an integrated approach to care management and community outreach, working closely with Pop Health RNs, medical providers, primary care teams, and various agencies to ensure patients are connected to appropriate care and resources, including primary care providers. How you’ll contribute A Supervisor, Care Navigator who excels in this role: Partners with Population Health leadership to realize the role of Care Navigators as part of an interdisciplinary team Supervises the work of a team of Care Navigators supporting patients by helping them navigate and access essential community services and resources. This includes providing direction and guidance to staff, monitoring staff schedules, and evaluating work for quality and completeness. Ensures assignment of outreach referrals and requests received are appropriate and delegated in a timely manner. Discusses challenging or problematic cases/situations with staff and makes suggestions and recommendations on best course of action to provide optimal services to clients/patients and community members. Makes recommendations to management for improved work methods, participates in workflow improvement processes and oversees implementation and operation of such recommendations and workflow improvement processes. Identifies gaps within CN programs and services and recommends solutions on how to creatively address them. Serves as an advocate for client/patient access to resources and services in the Clinically Integrated Networks and communities within. Identifies staff training needs and provides or coordinates training and coaching services to staff when needed. Reviews staff documentation to ensure compliance with program and departmental standards and workflows and provides suggestions for improvement. Interviews prospective personnel and make recommendations concerning their employment. Carries a caseload that includes more complicated client/patient assignments, as needed. Requirements

  • Exhibit a comprehensive understanding of healthcare regulatory and compliance (e.g. HIPAA).
  • Skilled in the ap-plication of policies and procedures.
  • Knowledge of Business Office Standards and Recommended Practices
  • Education: HS Diploma Required; prefer LPN or experienced MA
  • Experience: minimum of three years of experience in the healthcare setting with supervisory experience.

Nice To Haves

  • Ideal candidate will have prior experience in chronic care management, case management, clinic operations.
