This position is responsible to the Manager of Care of Management. This position is responsible for the day-to-day operations of Care Management including: Daily staffing scheduling; Supervising and mentoring staff; Assuring the standardization of care management; Assisting in responding to customer requests and problems; Maintaining staff performance informally through coaching, mentoring and providing developmental opportunities and formally through performance appraisal and disciplinary processes; Provides leadership/monitoring for compliance and performance and performance improvement.
