About The Position

As a Supervisor of Care Coordination, you will play a critical role in overseeing care coordination services that reduce barriers to care and improve patient health outcomes. This role provides day-to-day leadership, guidance, and oversight to care coordination staff while ensuring patients receive the tools, resources, and support needed to successfully navigate healthcare and social service systems. The Supervisor serves as a key link between clinical teams, social services, and community partners to support high-quality, patient-centered care.

Requirements

  • Licensed Master of Social Work degree (LMSW), Licensed Mental Health Counselor (LMHC) or related Master’s degree preferred or a bachelor’s degree with a minimum of 3 years of experience in providing direct case management services will be considered.
  • Experience supervising case management staff and performance based grant contract monitoring strongly preferred.
  • Ability to work within team-based structure.
  • Strong communication and interpersonal skills.

Responsibilities

  • Supervise a team of Patient Navigators, Case Managers, and Supportive Case Managers, providing guidance, support, and performance evaluations.
  • Provide direct case management services, including intake, assessment, treatment planning, and ongoing reassessment.
  • Coordinate care with medical providers, behavioral health professionals, and other community resources to ensure seamless patient care.
  • Assist patients in accessing entitlements and referring them to appropriate services.
  • Maintain accurate and timely documentation of all patient encounters and activities.
  • Monitor staff performance, provide feedback, and address any performance issues.
  • Participate in quality improvement initiatives and contribute to the development of best practices.
  • Attend conferences, trainings, and workshops to stay updated on the latest trends and best practices in care coordination.
