The LCSW Care Coordination Supervisor is a leader with sound clinical skills and actively assists the Leadership in coordinating both clinical and operational duties. The Supervisor oversees departmental staff including Transplant Social Workers in an effective and efficient manner. This includes creating staff schedules, daily assignments, vacation approvals, timecard management, contributing toward department goals, and participating in necessary disciplinary actions for staff. Supports the daily logistics of the Care Coordination department. Has extensive knowledge of department processes. Assists in selecting, training, performance review, and supervision as well as participating in Quality Improvement activities. Essential Duties: Coordinates and integrates various clinical resource management activities on intradepartmental, Interdepartmental and hospital wide levels. Takes an assignment when necessary to provide care to patients. Establishes and maintains performance improvement and quality control activities which support the Department and the hospital. Monitors staff’s clinical responsibilities in providing psychosocial assessment, interventions, and evaluation of the effectiveness of treatment plans in accordance with standards of practice and scope of care. Provides consultations to staff on difficult cases and provides a second opinion Keeps staff’s licenses, certifications, fire card and annual health assessment’s up to date. Plans for appropriate staffing, monitors, and adjusts as indicated by patient census, acuity or staffing changes. Creates and publishes daily Work Assignment by 8:00 AM Assists with staff evaluations. Plans for provision of services through appropriate levels of qualified, competent staff. Assists the Director in staff performance appraisals and performance management activities including monitoring attendance records. Performs document review audits for completeness, accuracy, appropriateness, and clarity of documentation by staff Supports and mentors staff. Manages escalations from staff. Provides for orientation, training and continuing education of staff. Actively assists with staffing transplant clinic and selection committees. Works with staff to ensure necessary information is transmitted to the Independent Living Donor social worker. Assists Leadership in establishing long and short-term goals for the Department, and remaining within acceptable variance of departmental operational and productivity budgets. Monitor space/resource requirements and utilization of outside resources. Assists with KRONOS time keeping. Proactively implements and maintains systems/protocols to improve operations. Completes eSRM reports. Is a team player. Displays a positive attitude. Performs other duties as assigned.
