Completes face to face assessments and healthcare services under the general supervision of the agency’s Medical Director. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all. Evaluates health status through performing comprehensive health assessments as required by agency policies. Accountable for completion of face to face assessments within required timeframes. Collaborates with physician, patient/family and interdisciplinary team regarding assessment findings, care needs, and revisions to plan of care. Establishes and maintains positive working relationships with patients, family members, co-workers, physicians and referral sources. Performs timely and accurate documentation in accordance with agency policy. Supervises Medical Services Advanced Practice Practitioner staff administratively by monitoring expenses, managing timekeeping and time off requests, completing the Medical Services Department on-call calendar, and assisting with other administrative duties as needed. Abides by the Health Insurance Portability and Accountability privacy and security regulations regarding all aspects of Protected Health Information (PHI).
