About The Position

The SUD Outreach Clinician is accountable to the Associate Director for providing treatment and casework services to the clients and families to which they are assigned. RESPONSIBILITIES: Serve as the primary Clinician to develop a person-centered treatment plan that focuses on addressing the needs of the client and family engagement when applicable. Work with the client, family, and other involved individuals, i.e. school personnel, probation officer, mentor, DHHS, medical personnel, etc. For the purposes of coordination of care, follow community partner, PIHP, CMH, and agency guidelines when providing treatment with patients receiving Medication-Assisted Treatment or other medical treatment. Provide education to clients, family, and other involved parties on MAT. Provide individual, family therapy, and case management as needed to each client/family on their caseload, with a minimum of one to two hours face to face contact each week. Coordinate the transfer/discharge process for the client and family, informing the Associate Director and the Admissions Department of projected transfer/discharge planning. Continuous Marketing of SA and Community programs, facilitate communication and good working relations between the agency and the community, including police, church, neighbors, volunteers, employers, medical personnel, and friends of individual clients. Complete all service plans, contact records, and all other necessary paperwork, in a timely manner. Develop Service Plans, Discharge Summaries, progress notes, and other documents as required by contract. Monitor the quality of overall treatment services to clients on their caseload; and evaluate the adequacy of these services to meet treatment objectives. Also, administers and assists in the coordination of GAIN and GPRA measures. Participates in regularly scheduled staff meetings, therapist meetings, and any others as deemed necessary by the Community Leadership. At least twice a year represent Wedgwoods Community Programs to market services in the community (events, health fairs, etc.) Represent Wedgwoods mission and vision with community partners. Exercise excellence and integrity as the job requirements are fulfilled. Respect others and understand their needs and feelings, regardless of race, social, ethnic, economic, and religious background; have an attitude and aptitude for providing nurturance, being patient, and accepting others. Professional Growth Possess a working knowledge of licensing and accreditation standards and work with other disciplines to assure compliance with such standards. Utilize educational opportunities and resources offered by the agency and through the profession for continued development of skills and clinical expertise, (conferences, in-services, clinician/consultant meetings, resource groups etc.) Assist with the professional growth of new hires by allowing job shadowing as needed. Attend peer supervision meetings and clinical consultation meetings as scheduled. Responsible for quality assurance compliance, including, but not limited to, participating in peer supervision, peer utilization reviews, compliance with service delivery criteria and reporting requirements for each individual served. Requirements

  • Bachelors degree in Social Work, Psychology or another associated human service profession.
  • For transportation purposes must have a valid driver's license.
  • Subscription to the Statement of Faith of Wedgwood Christian Services and agreement with the goal, purpose and objectives of the agency.

Nice To Haves

  • Clinical or case management experience working with adolescents and/or families in home based setting preferred.
  • Prior experience working with an interdisciplinary team preferred.
  • Registered Development Plan from Michigan Certified Board for Addiction Professionals (MCBAP) required upon hire.
