The Success Coach (Case Manager) for Targeted Case Management (TCM) will provide specialized case management services to individual client needs. They also create programs to benefit clients and educate clients on available community resources. Success coaching ensures that clients receive services in a timely, appropriate, efficient and coordinated fashion. This position requires that in-person support and/or service delivery be provided by the employee in various settings approved by organizational leadership to include, but not limited to: homes, schools, outdoor settings, courtrooms, offices, shared spaces, street outreach locations, etc. These environments are often inclusive of higher levels of noise and other factors that may present a heightened level of stimuli and safety awareness. When providing or participating in leadership-approved on-line activities, being on-camera and actively engaged are also expected and required as part of this work.
