About The Position
The position is responsible for case management and service coordination of post permanency services through direct facilitation of Success Coach services to youth and families who have achieved permanency through adoption, reunification, custody, or guardianship. Success will be accomplished by increasing placement stability, decreasing reentries into foster care, promoting positive well-being of youth who have experienced foster care, and maintaining fidelity to the model.
