About The Position

Being a Substitute Direct Support Professional is more than just helping someone with their daily activities; it is assisting someone to live a meaningful life. In this role, you will be a mentor, friend, and advocate for the individuals you support. Essential Responsibilities and Job Duties: Provide direct care and supervision for people with intellectual and developmental disabilities in the community and in their personal home. Assist individuals in completing living skills, money management, cooking, and community participation, hygiene routine, household cleaning, and grocery shopping Provide transportation to individuals as needed. Assist with medical care (medication administration, and transportation to medical appointments). Implement and document individual goals and medical tracking electronically. Coordinate other resources as needed including support in arranging and/or directly providing transportation for participants. Facilitate and monitor activities consistent with Individual Service Plan goals and objectives for assigned caseload. Responsible for supporting and maintaining a physically and psychologically safe environment. Must keep track of their own sub shift schedule. Eligible to sub with the day program, if cross-trained with day program team, but must not abandon a previously registered shift for a more desirable one. Provide overnight on-call support for emergency response. Expectations of Position: Able to work well with team members in all departments within Wellspring Community and the public at large. Well-developed communication skills, both written and verbal. Demonstrates ability to manage challenging behaviors through the execution of positive behavior supports for each individual/unique situation or seeks help from senior positions when necessary. Excellent time management skills

Requirements

  • High School Diploma or Equivalent required.
  • Experience working with the IDD community or other at-risk population
  • Experience working with Electronic Health Records (EHR)
  • Valid Driver’s license
  • Proficiency in computer skills including Microsoft Office and Electronic Charting Systems.
  • Must be able to pass a background check, drug screen, and obtain DOT certification.
