Substance Abuse Specialist is a bachelor’s level paraprofessional with at least 40 hours of substance abuse course work, CADC preferred and clinical experience providing assertive outreach, services linkage, substance abuse counseling, resources, benefits assistance, referral, and housing services for homeless adults with behavioral health problems. The Substance Abuse Specialist is responsible for facilitating program admissions and discharges, for monitoring client eligibility, and for ensuring that appropriate services/service linkages are provided to each person served. S/he is responsible for case planning, utilization management, data collection, and continual assessment to ensure that persons served are receiving the right level of care to meet their needs, and that services are tracked using the approved management information system.
