About The Position

The Substance Abuse Case Manager provides monitoring services for the Health Practitioners’ Monitoring Program (HPMP) for participant adherence to their individualized monitoring contract. The Substance Abuse Case Manager manages all aspects of participant progress through the program including development and implementation of monitoring contracts, ongoing monitoring adherence to contract, and administrative documentation of all aspects of program progress. Requirements

  • Minimum of two (2) years of work experience in chemical dependence treatment and/or mental health treatment
  • Bachelors Degree in Counseling, Social Work, Nursing, Psychology or related field from an accredited program
  • Physical Lifting less than 20 lbs.
  • Activities Prolonged sitting
  • Mental/Sensory: Strong recall, Reasoning, Problem solving, Hearing, Speak clearly, Write legibly, Reading, Logical thinking
  • Emotional: Fast pace environment, Able to handle multiple priorities, Frequent and intense customer interactions

Nice To Haves

  • Licensed Professional Counselor or Certification as a Substance Abuse Counselor preferred

Responsibilities

  • Provides monitoring services for the Health Practitioners’ Monitoring Program (HPMP) for participant adherence to their individualized monitoring contract.
  • Manages all aspects of participant progress through the program including development and implementation of monitoring contracts, ongoing monitoring adherence to contract, and administrative documentation of all aspects of program progress.
  • Routine interactions with participants, HPMP staff, and Department of Health Professions’ staff.
  • Supervises the daily assignments of the Case Management Assistants.
