Eliot is seeking a strong, experienced Substance Abuse Case Manager to join our Program for Assertive Community Treatment (PACT) team. PACT is an evidence-based model that provides comprehensive treatment, rehabilitation, and support to adults with severe and persistent mental illness. The ideal candidate will demonstrate a strong ability to develop and facilitate dual diagnosis groups, as well as provide individualized substance use treatment and support. A thorough understanding of twelve step programs is required. Candidates must also possess demonstrated knowledge of principles related to consumer empowerment and recovery-oriented care. Requirements

  • Requires a Bachelor's degree in Psychology, Social Work or related field
  • Experience working with adults with substance abuse disorders
  • Valid Massachusetts drivers license

Responsibilities

  • Act as primary case manager for identified clients, providing targeted support and interventions.
  • Provide individual and group substance use treatment and support.
  • Participate in assessments and treatment planning with clients and the team.
  • Ensure consumer safety by implementing crisis intervention techniques when necessary
  • Utilize a team based approach to support and services, collaborating with colleagues and actively participating in daily meetings.
  • Participate in rotating on-call coverage and evening shifts to ensure 24/7 support for clients
