About The Position

Arizona Families F.I.R.S.T. (Families in Recovery Succeeding Together- AFF) helps parents address substance abuse issues that are affecting their ability to care appropriately for their children or to get and keep a job. It provides the opportunity for families to overcome the barrier of substance abuse to reach the outcomes of permanency for children, family reunification and self-sufficiency. The goal of the program is to reduce or eliminate abuse of and dependence on alcohol and other drugs and to address other adverse conditions related to substance abuse. JOB SUMMARY: The Substance Abuse Case Coordinator serves individuals referred to Catholic Charities’ Arizona Families F.I.R.S.T. program (Families in Recovery Succeeding Together – “AFF”) which supports parents address substance abuse issues that are affecting their ability to care appropriately for their children. For clients in the JOBS program, it will assist clients to get and keep a job. Responsible for providing comprehensive case management including leading groups in discussion to assist families in overcoming the barriers of substance abuse to reach permanency for children, family reunification and self-sufficiency. This position is a telecommuter position.

Requirements

  • Minimum of a High School Diploma or equivalent with at least 3 years of related experience including working with multi-problem families in the field of substance abuse, child welfare, or behavioral health.
  • Must have knowledge and understanding of human behavior, social work theories and techniques as well as community resources.
  • Must have counseling, interviewing assessment, therapeutic relationship skills.
  • Must be flexible and able to work with the uniqueness of each client.
  • Must be sensitive to cultural differences within the agency and community.
  • Fluent in Microsoft Office Applications (Word & Outlook).
  • Must be able to stoop, bend, squat, sit on floor and ambulate over uneven floor surfaces including stairs and steps.
  • Must be able to visually and auditory assess and interact with clients to ensure their safety and wellbeing.
  • Requires considerable work outside of the office and substantial driving; requires evening and weekend work on occasion and willingness to be on-call for emergencies; may lift up to 25 pounds.
  • Valid AZ driver’s license, reliable transportation, and ability to show proof of registration/insurance.
  • Level One Fingerprint clearance or ability to obtain fingerprint clearance; must maintain clearance.
  • You must be 21 years of age or older to drive on behalf of Catholic Charities.

Nice To Haves

  • An Associate’s degree in a human service or behavioral health-related area of study preferred.

Responsibilities

  • Responsible for receiving referrals and engaging clients within 24 hours, through home visits or telephone contact. Completes documentation regarding all contacts and information regarding the case.
  • Responsible for continued outreach and engagement of clients through telephone calls, home visits, and ongoing communication with all members of the substance abuse treatment team.
  • Oversees the collection and review of all collateral information including but not limited to: social histories, psychological evaluations, judicial records, previous substance abuse treatment information, DPS reports, and obtaining a release of information for everyone involved in the case.
  • Provide on-going support and case management expertise to those experiencing substance use.
  • Work with clients to develop individualized case management plans.
  • Coordinates substance abuse assessment including family centered interviews and all required substance abuse tools.
  • Responsible for developing and implementing individual service plans, making community referrals, on-going reassessment of the service plans and follow up. Updates service plans bi-annually or as needed.
  • Coordinates all services for each individual client, including advocacy, giving information about community resources, etc.
  • Provides accurate documentation of services, including identifying resources, writing progress notes, detailed summaries of all services and contracts and submits as required or outlined. Completes monthly progress reports and billing as required.
  • Facilitate Intensive Outpatient and Outpatient Groups utilizing evidence-based curriculum.
  • Advise and support individuals and groups with substance abuse issues.
  • Responsible for completing appropriate assessments tools; American Society of Addiction Medicine (ASAMs), Addition Severity Index (ASI), Clinical Institute Withdrawal (CIWA), Clinical Institute Narcotic Assessment (CINA) and ACES within allotted deadlines.
  • Coordinate, facilitate and participate in Adult Recovery Team (ART) meetings with multiple stakeholders.
  • Promote ongoing commitment to excellence and continual quality improvement to ensure the highest quality of services to our staff and clients.
