This position is staffed 12 months of the year. Student Health Centers (SHC) is a unit within Integrated Clinical Services (ICS). The mission of SHC is to facilitate access to comprehensive preventive primary health care and integrated behavioral health services for Multnomah County school-age youth to keep them healthy and ready to learn. The program operates 9 centers in coordination with five school districts: Centennial, David Douglas, Parkrose, Reynolds and Portland Public Schools, and the School Based Mental Health Program of the Multnomah County Behavioral Health Division. The Advanced Practice Clinician provides professional, comprehensive specialized nursing services, assessment and evaluation with an emphasis on prevention of disease and promotion of health. Establishes a therapeutic relationship with a diversity of patients, including refugees and low-income population groups. The Advanced Practice Clinician also contributes to creating a work culture of respect, trust and understanding for the highly diverse populations the Health Department serves in the office and in the community. As the Advanced Practice Clinician, other duties include: Clinical Care Providing low barrier healthcare Performs comprehensive health assessments and clinical procedures by completing a physical and psychosocial history Identify actual and potential health needs based on medical, behavioral, financial, and social factors. Diagnoses and treats acute and chronic medical conditions according to scope of practice to include conducting a medical history, conducting physical exams, ordering and interpreting diagnostic tests, devising a course of treatment, discussing alternative treatment, risk and benefits of treatment, prescribing and monitoring medications, appropriate counseling of patient and as needed, family members, and reassessment of the patient at appropriate interval. Utilizes team to prioritize and direct care around annual comprehensive health screenings, preventive health practices, chronic disease management, community resources and mental health needs of clients. Deliver care to remote clinics using telemedicine technology and workflows. Outreach to school partners Documentation Completes patient documentation and manages electronic in-basket according to established policies and procedures. Documents actions taken per client, staff assignment of duties, etc. Review and provide feedback to leadership on internal and external policies and procedures that affect client healthcare delivery and services for improvement and/or correction within policies or procedures. Manages confidential health information as part of chart notes and documentation to set care plans for other providers and staff to provide team approach healthcare delivery. Completes patient documentation and manages electronic in-basket according to established policies and procedures. Referrals Assesses and counsels patients regarding preventive care needs; establishes need for specialty and rehabilitative services; delivers services or makes appropriate referrals. Assesses and counsels patients regarding preventive care needs; delivers services or makes appropriate referrals. Determines need/schedule for high risk referrals. Notes referrals and outcomes in documentation for care plan and charges/reimbursement of services. Consultation/Coordination Seeks clinical consultation as needed. Provides consultation to support staff for client care plans and services to be delivered. Be an active member of an integrated clinical team Coordinates with Behavioral Health providers to effectively integrate care and provides consultation to primary care providers, nursing staff, medical assistants and other behavioral health team members regarding patient behavioral health concerns. Provides care management and linkage services including specialty health referrals for clients for whom providing treatment and care. Provides coverage for clinical colleagues in their absence. Requirements

  • Nurse Practitioner License, Registered Nurse License or Physician Assistant
  • National Board Certification through AANP, ANCC, or NCCPA
  • DEA Certification
  • BLS / CPR Certification
  • Successful completion of Background check
  • Spanish speaking - Pass Spanish language test
  • Training and experience for this position is Board Certification and State licensure as a Nurse Practitioner or Physician Assistant and one (1) to four (4) years of experience that demonstrates the ability to perform the duties of the position.
