About The Position

This position performs identification, assessment and management of high risk individuals with structural heart conditions requiring application of management concepts and evidence-based guidelines.

Requirements

  • Bachelor's Degree, required
  • BSN required
  • Minimum of five years experience in disease/condition management programs or relevant clinical experience in the management of cardiac patients, required
  • Current RN license in appropriate state, required
  • Knowledge of structural heart necessary to obtain and interpret information in terms of the patient's needs
  • Knowledge of Medicare/Medicaid rules/regulations necessary to understand criteria for admission
  • Knowledge of nursing and pharmacy procedures and standards of practice, both general and specific, to the specialties of structural heart
  • Ability to modify care according to patient's needs and ability
  • Ability to work in collaboration with Physicians, Anesthesia services, and other interdisciplinary team members
  • Strong ability to educate patients on what is required to maintain good health
  • Ability to effectively collaborate with teams, groups, and committees
  • Demonstrate leadership ability
  • Ability to prioritize and organize
  • Ability to interact effectively with diverse individuals at all levels of authority
  • Must be flexible, adaptable to change, and able to apply problem-solving skills and critical thinking skills in meeting patient care and department needs
  • Must have computer experience
  • Must have reliable transportation to and from all work-related assignments and activities

Responsibilities

  • Completing condition specific assessments of risk and creating a plan of care with the team for treatment
  • Interpreting and relaying diagnostic test results to participants
  • Providing telephonic coaching and teaching
  • Educating participants on behavioral change towards improving health status, reducing health risks, and improving quality of life
  • Scheduling and follow-up with participants as needed with appropriate documentation on all participants
  • Deliver the most remarkable patient experience in every dimension, every time
  • Foster teamwork, team member engagement, and community involvement
  • Commit to leveraging diversity and inclusion in support of quality care
  • Foster a safe patient environment driven by the principles of 'First Do No Harm'
