About The Position

The Specialty Care Coordinator I assesses, plans, implements and coordinates treatment processes from diagnosis through follow up, specifically guides patients through the collaborative process with all medical care providers and ancillary support programs and provides educational support along the continuum. Job Essentials 1. Provides patient and family education and resource assistance. Links with community/outside agencies as an educator and resource. Demonstrates effective presentation and communication skills including interpersonal communication delivered with warmth and sensitivity to treatment. Assists with the development, implementation and operation of patient education and facilitation of follow-up care. 2. Provides clinical care and rounding within scope of practice. Organizes and analyzes data to develop an individualized, comprehensive plan of care and follow up. Collaborates with all disciplines in forming a timely and appropriate plan of care and links with clinical trial programs for information and clinical trial participation. 3. Assists in the development and implementation of quality/safety/patient experience improvement activities to include patient satisfaction monitors and quality of life monitors. Provides clinical expertise in the specialty area and is a pathfinder in the identification and development of quality patient care delivery. 4. Monitors and evaluates trends and changes in clinical, service, and cost outcomes to various interventions. Utilizes research to improve nursing practice and to stay abreast of national and regional standards, diagnosis and treatment. As applicable, responsible for maintaining certification of program. Support for achieving and/or maintaining Joint Commission accreditation and compliance with other federal and state regulations. 5. Engages with Providers at the service-line level to achieve clinical, operational and service goals as appropriate. Acts as a resource and primary contact for providers in the department. Enhances provider productivity by proactively identifying and removing operational barriers and responds to provider's operational needs.

Requirements

  • Bachelor's degree in Nursing (BSN). Education must be obtained from an accredited institution. Degree will be verified.
  • Current RN licensure in the state of practice.
  • BLS certification for healthcare providers.
  • Unit specific certification as required (i.e., ACLS, PALS, etc.).
  • Basic computer skills.
  • 2 years of clinical nursing experience.
  • Experience in assigned clinical area.
  • Leadership and mentoring experience.

  • Well-developed interpersonal skills with the ability to develop excellent working relationships with providers and ancillary departments.
  • Demonstrated experience with quality management programs including registry data collection.
  • Computer skills including the creation of graphs, tables and trend reports.
  • Demonstrated critical thinking skills, data organization, management, and analysis skills.
