The Structural Heart Coordinator manages the care of patients with structural heart and valve disorders across the continuum in collaboration with the Cardiothoracic Surgeons, Cardiologists and other members of the multidisciplinary team. The Structural Heart coordinator will be responsible for coordinating comprehensive, specialized care for structural heart and valve disorder patients (TAVR, TMVR, Mitra Clip, TTVR.). The Structural Heart coordinator will function as a central figure in the overall and daily operations of the structural heart and valve disorders program, and other advanced cardiac valve procedures according to CMS guidelines, identifying inclusion and exclusion criteria. The Structural Heart coordinator will perform advanced assessment, planning and coordination of care, modification of the treatment plan as indicated, and the monitoring and evaluation of outcomes outlined by attendings. Coordinates baseline and follow up visits and testing according to established criteria. Works closely with physician leadership to ensure accurate ACC NCDR registry data obtained to track the quality and success of the program. Initiates consultations/referrals and maintains consistent follow up care in collaboration with referring and/or primary care physicians. Participates in outreach activities with participating surgeons and cardiologists to build the clinical practice and referral patterns. This position will directly report to ACU Manager of UF Health Heart and Vascular and indirectly to the Chief of Cardiothoracic Surgery and Chief of Cardiology.
