About The Position

The Structural Heart Coordinator coordinates all services/systems required for an organized multi-disciplinary team approach to the evaluation, treatment and follow-up of the patient needing structural heart procedures, including TAVR, Watchman, Mitra Clip and other exciting new procedures. This position reports to the Structural Heart Program Manager and works under the direction and guidance of this manager. Requirements

  • Licensure as Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing.
  • Baccalaureate in Nursing with current license to practice as a Registered Nurse in the State of Rhode Island.
  • Certified in Basic Cardiac Life Support (BLS).
  • Certified in Advanced Cardiac Life Support (ACLS).
  • Two years related experience as RN in cardiac care preferred.

Responsibilities

  • Participate in all visits (consults, and f/u)
  • In collaboration with MD and APP, conducts assessment of patient needs by evaluating/interviewing all new/returning patients.
  • Triages patients to appropriate care resource (includes direct contact and telephone encounters).
  • Reviews patient history and other supporting clinical data to facilitate optimum care.
  • Complete nursing assessments including nursing history, symptom assessment, medication history and reconciliation.
  • Organizes care conferences with the multi-disciplinary staff during the screening process and promotes a team approach to the patient program.
  • Effectively explains and educates patients and/or family members about the procedures.
  • Teaches based upon assessment, expressed need from patient and/or family or the initiation of new treatment.
  • Communicates the plan of care and other pertinent information to/from other healthcare providers.
  • Completes patient education.
  • Conducts TAVR eval screens-KCCQ, Frailty, 5-meter walk test.
  • Acts as a resource to schedulers for questions, problem solving.
  • Compiles and reviews all diagnostic testing and labs.
  • Coordinates with pre-surgical testing services as needed.
  • Demonstrates a fundamental grounding in nursing theory and practice with mastery of knowledge within a defined content area.
  • Manages a caseload of patients in partnership with the healthcare team to optimize health status and coordinate care over time and across healthcare settings.
  • Provides leadership in the development, validation, implementation, and evaluation of standards, policies, procedures, and critical paths to support the patient care delivery model.
  • Assists the healthcare team to achieve mutually agreed upon healthcare goals by providing necessary support, education and encouragement.
  • Provides expert consultation for clinical issues promoting increased knowledge and skills for the consultee.
  • Participates in the education, clinical research and mentoring of healthcare providers.
  • Facilitates patient/family education in area of expertise.
  • Annually completes mandatory safety infection control and BLS.
  • Performs other duties as assigned.
