About The Position

Registered Nurse (RN) – Structural Heart Clinic - Valve Coordinator Full time 40 Hours a week Monday through Friday. Closed on weekends and holidays. Hours can be flexible based on department need. The Valve Coordinator works collaboratively with the cardiovascular surgeons and cardiologist to provide quality and consistent care to Valve Disease patients. Responsible for coordinating the movement of patients through the referral and evaluation process, pre-valve treatment management, scheduling and evaluation process, and post valve treatment care. Responsible for identifying the learning needs of pre and post valve disease patients, implementing teaching plans appropriate to the patients’ level of understanding, and documenting plan and progress in patient charts. The Coordinator is responsible for coordinating the patient’s evaluation and to establish and maintain good relationships with referring cardiologists and clinics in the service area to promote increased patient referrals. Responsible for any duties as designated by his/her supervisor to improve clinical services and promote quality cardiovascular services.

Requirements

  • Bachelor’s Degree in Nursing.
  • 2+ years intensive care and/or cardiac telemetry experience which includes Knowledge, Skills & Abilities Required:

Responsibilities

  • In collaboration with the cardiovascular surgeons and cardiologist, plans, assesses and implements the valve disease patient evaluation, peri, and post operative care as it relates to valve disease patients.
  • Participates in the initial evaluation and education session of patients and their families considering valve disease treatments.
  • Develops a nursing care plan for pre and post valve repair treatment which includes patient and family education, and psychosocial needs.
  • Under the established medical protocol, initiates the medical evaluation of the valve disease patient which includes: coordination of diagnostic testing per valve evaluation guidelines, referrals to physician and specialists, obtain and review patient medical records.
  • Assesses the valve disease patient evaluation results, and communicates to CV surgeons and cardiologist
  • In collaboration with Advanced Practice Nurses (APNs), organizes, prepares and presents valve disease patients to the multidisciplinary valve Committee
  • Provides continuing education and psychosocial support to the patient and families during the pre and post evalutation process.
  • Provides outpatient support to physicians and APN's. In collaboration with physicians and APNs, provides support with patient encouters/visits in the outpatient clinic, reviews and provides follow-up on diagnostic testing, laboratory results, and communication with consultants and the valve team.
  • In collaboration with physicians and APNs, schedules procedures or testing, and provides follow-up communication to the patient on the above testing
  • Coordinates all short and long-term followup care for patient, including but not limited to 30 day and 1 year follow up.
  • Upon valve evaluation completion, works collaboratively to place patients on the list for surgery or procedure.
  • Works collaboratively with other APNs, nurses and data coordinator to maintain/update lists
  • Maintains communication with the patient regarding patient’s medical and follow up plan of care.
  • Serves as a resource person for other members of the multi-disciplinary health care team (e.g., pharmacist, surgery and lab personnel). Develops and maintains professional relationships with the Advocate System and other area clinics
  • Assists surgeon and cardiologist in maintaining communication with the patient’s referring cardiologist during the evaluation phase and post-intervention.
  • Does necessary data entry of all valve patients’ work-up, labs, and other test results in the STS and TVT databases.
  • Collaborates with APNs, data coordinators, nurses, and administration in remaining complaint with the required transcatheter valve regulations of CMS. Participates in regulatory administrative audits and remains up to date on CMS, STS, and TVT regulations
  • Collaborates with surgeons/cardiologist/research nursing with any valve related research trials or registries.
  • Coordinates systematic valve disease screening protocols in collaboration with the physicians and echo department.
  • Serves as a consultant and/or resource person for hospital personnel in area of specialization. Accepts referrals from all units of the hospital and collaborates with physicians, nurses and other members of the multidisciplinary team in planning the care of patients needing specialized valve care. Initiates referral to other healthcare providers and resource agencies as necessary.
  • Provides formal and informal education to the outside health care personnel and community groups.
  • Identifies learning needs of patients and families and implements teaching plan as appropriate as it relates to patients.
  • Contributes to the development of patient education/marketing materials
  • Coordinates and attends regular multidisciplinary valve team meetings
  • Works on special projects as assigned.
  • Acts as mentor/clinical preceptor for nursing students.
  • Maintains the standards and educational requirements specific to the position through continuing education.
  • Serves as representative in external committee and organizations.
  • Continuously updates knowledge of current developments in patient care management, health care legislation and hospital/community resources
