About The Position

The Stroke Program Navigator is responsible for facility's stroke education organization and compliance while serving as subject matter expert and resource to clinical staff, under supervision by Stroke Program Coordinator and Quality Director. Concurrent review of stroke patients throughout hospitalization and after discharge, educating both patient, family, and nursing staff to maintain compliance with stroke center requirements.

Requirements

  • Generally requires 5 to 7 years of related experience.
  • Bachelor's degree in Nursing is required.
  • Valid Florida RN license is required.
  • SCRN or obtain SCRN within first year of employment.
  • American Heart Association Basic Life Support (BLS) and any additional applicable life support certification for Healthcare Providers is required upon hire with at least 6 months validity and maintenance at JHS for the duration of employment.
  • Must meet and maintain valid and current all unit specific and organizational skills/competencies, certifications/licensures, as required by regulatory and/or nursing standard of practice for the specialty.

Responsibilities

  • Works with Stroke Program Coordinators and other stroke team staff to chair corporate stroke required education committee ensuring all required education is aligned, reviewed and compliant annually with TJC regulatory guidelines.
  • Coordinates and prepares planned on-site stroke service evaluations, designations and certifications.
  • Monitors compliance with TJC assessment and reassessment requirements.
  • Assists stroke team staff in problem solving related to the care of stroke patients, equipment, procedures, rehabilitation, and discharge plans.
  • Coaches staff to develop, maintain, and enhance clinical skills, independent problem solving, systems thinking, and decision-making.
  • Just-in-time education to be provided when knowledge gaps are revealed.
  • Creates, evaluates, delivers, and coordinates formal and informal intra and inter disciplinary educations to improve patient outcomes and quality of care.
  • Develops education for secondary prevention and coping strategies for patients and caregivers via stroke support group and community outreach.
  • In collaboration with Stroke Program Coordinator to provide education to providers from EMS and interhospital, i.e. hospitalists, residents.
  • Responsible for ensuring appropriate education is assigned/provided to physicians, midlevel providers, nurses, etc. and that community outreach education meets each hospitals regulatory requirements based on facilities certifications and accreditation.
  • Participates in committee meetings requiring stroke representation throughout the hospital.
