About The Position

The Nurse Case Manager demonstrates a high level of expertise in the assessment, diagnosis, and treatment of the complex responses of individuals, families, or communities to actual or potential health problems; prevention of illness and injury; maintenance of wellness; and provision of comfort.

Requirements

• Current RN licensure in Virginia or eligible or compact state.
• AHA BLS HCP Certification or equivalent.
• Three (3) years of experience including two (2) in a specialty area.
• Baccalaureate Degree in Nursing from an accredited School of Nursing.

Nice To Haves

• Two (2) years case management/care management experience.
• Certification in specialty area as appropriate.

Responsibilities

• Makes appropriate decisions and initiates appropriate interventions in the provisions of care coordination within the boundaries of professional practice, the Standards of Practice for Case Management (CMSA) and established policies.
• Organizes and plans work with input from the client/family with specific outcomes and demonstrated use of sound clinical judgment and resources.
• Determines the appropriate place of contact/intervention with customer including client’s home, community agency, provider office, etc. in order to achieve customer satisfaction and a positive outcome.
