About The Position

RN, Street Medicine Position Purpose: The Registered Nurse Community Health & Well-Being, within the scope of Ohio Nurse Practice Act, will provide care in an atmosphere sensitive to each person's physical, emotional, social, and spiritual needs. Accountable for the delivery of patient care utilizing the nursing process within the standards, policies, procedures, and guidelines of the Organization. This position has responsibilities in three key areas: clinical, case management, and supportive administrative tasks (ie. creating the monthly team schedule, preparing for flu shot clinics, etc.). This nurse will join providers on street rounds and provide patient care in homeless camps. This nurse works some mobile clinic sites and serves as the back-up nurse if another nurse is ill or on vacation. This nurse carries a nurse cell phone and addresses patient inquires and needs during business hours. This nurse will assign providers to follow-up with patients with medical needs based upon inquires received on the nurse cell. Incoming calls may warrant some short-term case management to connect patients to shelter, specialty care, medications, and other social resources. This nurse, also, manages the Epic inbasket for the Street Medicine providers. Administrative tasks involve creating the monthly team schedule, following up on patient referrals within Epic, planning special events, and other tasks that support the program manager.

Requirements

  • Graduate from school of nursing, BSN preferred
  • Current license to practice as a Registered Nurse in the State of Ohio.
  • Must have a valid Driver's License.
  • BLS Healthcare provider training required
  • Requires use of a personal vehicle with valid auto insurance.
  • Minimum of 3 years’ experience

  • Other tasks that support the program manager.
