Position Description: The Street Medicine Case Manager is responsible for conducting field-based (ie: at tents, in encampments, at community spaces, at SROs) assessments of clients’ needs, including conducting psychosocial and cognitive assessments; developing and continually updating collaborative client care plans; providing referrals to health and psychosocial service resources and programs; and providing informal, field –based short-term psychosocial counseling to address immediate client barriers and impediments to care, including mental health and substance abuse issues. The Case Manager will be a provider in our Enhanced Care Management (ECM) program and will document accordingly. The position is also responsible for the orientation of new staff, interns, and volunteers, and coordinating services within their program as assigned by their direct supervisor. This position will be active with other SFCHC Programs and participate when possible, with SFCHC Committees and Events. Direct client work and interaction will be done in the field (tent-side, encampments and in community spaces) and at the office.
