About The Position

West Oaks Hospital has provided psychiatric care to the Houston area and surrounding communities for over three decades. Our 176-bed acute care facility is fully accredited and included a 16-bed residential treatment center for adult chemical dependency. We offer a broad range of behavioral health programs for children, adolescents and adults. Treatment is offered at multiple levels of care including inpatient, partial hospitalization and intensive outpatient. We are available 24 hours a day, seven days a week to assist you and answer your questions about treatment options and insurance coverage. Visit us online at: https://westoakshospital.com/ West Oaks Hospital is seeking a dynamic and talented Step-Down Navigator to join our team of compassionate, dedicated professionals. The Step-Down Navigator is responsible for meeting with all eligible patients to discuss the benefits of stepdown services as part of their continuum of care treatment. The Step-Down Navigator will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to eligibility, into Midas for trending report purposes. The navigator will have a thorough understanding of the step-down process and will coordinate between departments.

Requirements

  • Education Required: High school diploma or GED.
  • Licensure and certification Required: None Licensure. CPR and hospital de-escalation certifications. Preferred: RN, LVN, Licensed Counselor or Social Worker.
  • Experience Required: One year of clinical experience in an inpatient psychiatric setting. Through understanding of the step-down process familiarity with involved department.
  • Thorough understanding of the Inpatient workflow process, as it pertains to case management and discharge planning.

Nice To Haves

  • Preferred: RN, LVN, Licensed Counselor or Social Worker.
