Parkwood Behavioral Health System (a UHS facility): Parkwood Behavioral Health System provides behavioral health and chemical dependency services to the Mid-South region and has been in business since 1987. For more information about Parkwood Behavioral Health System, please visit our website at https://parkwoodbhs.com/ Step-Down Navigator Full-time (Hybrid) The Step-Down Navigator is repsponsible for both internal and external objectives. The step-down navigator will meet with all internal eligible patients to discuss the benefits of Outpatient services as part of their continuum of care treatment. The Step-Down Navigator will also be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to Outpatient eligibility, into Midas for trending report purposes. The Step-Down Navigator is responsible for meeting with potiental external referral sources within a 35 mile radius.

Requirements

Bachelors's Degree required in marketing or human services field.
Minimum two (2) years' experience in psychiatric/mental health field.
CPR Certification required
A thorough understanding of the Inpatient workflow process, as it pertains to case management and discharge planning, is preferred and business development techniques.

Responsibilities

Meet with all internal eligible patients to discuss the benefits of Outpatient services as part of their continuum of care treatment.
Communicate eligibility to key members of senior leadership & associated departments
Routinely collecting and entering data, specific to Outpatient eligibility, into Midas for trending report purposes.
Meeting with potiental external referral sources within a 35 mile radius.
