About The Position

We are hiring for: START Therapeutic Coaching Type: Regular If you are a positive and personable individual looking for a satisfying and fun opportunity to make a real difference in the lives of people with intellectual, developmental disabilities, and people facing mental health, and substance use challenges, join our team at RHA Health Services! Serves as the primary contact supporting individuals in the coaching program with Developmental Disabilities and Mental Health challenges for the START Program (Systematic, Therapeutic, Assessment, Resources, and Treatment) to provide community based intervention and prevention services to active participants within START. The program relies on integrated care and treatment with collaboration among all team members. The services in the START Program include collaborative contacts, after hour contacts, emergency team meetings, therapeutic coaching and therapeutic respite. We are Looking for a Therapeutic In-Home Coach to Join our Team for our START Program!!!!!

Requirements

  • Bachelor’s degree in psychology, Social Work or other Human Service related field required.
  • 2 years’ experience with population served preferred.

Responsibilities

  • Coaches work within the home or other community settings to create and implement person specific and crisis therapeutic interventions and communicate these interventions with primary caregivers.
  • Contribute to the overall crisis stabilization and prevention of the individual and family by providing hands-on assistance and education to families.
  • Participate as part of comprehensive START services that collaborate with individual's primary provider and clinical home, community crisis services, family and others.
  • Offer consultation and hands-on education with entire family, support system to create a safe, nurturing, and meaningful environment for families and individuals with development disabilities.
  • Assist individuals with implementation of daily activities, behavioral health needs, and health, wellness, and leisure activities.
  • Monitor individuals for any mental health or behavioral symptoms and provide crisis prevention, intervention and de-escalation support in accordance with crisis guidelines and person's plan.
  • Maintain timely documentation of session and visits notes and other data.
  • Work with families and individuals to work on identified needs/skills to assist with strategies to improve coping mechanisms.
  • Help to develop strategies and tools to assist others with improving overall well-being.
