Dependable Health of Nevada is seeking a full-time Start of Care (SOC) Home Health Nurse to join our growing team in the Las Vegas area. Position Overview The Start of Care Nurse is responsible for completing initial home health assessments, initiating plans of care, and ensuring accurate and timely documentation in compliance with Medicare and agency standards.

Requirements

• Active Nevada RN license (required)
• Home Health experience required
• Strong knowledge of OASIS and Medicare guidelines
• Excellent assessment, documentation, and time-management skills
• Valid driver’s license, reliable transportation, and current auto insurance
• Ability to work independently while maintaining strong communication with the care team

Responsibilities

• Complete Start of Care (SOC) and Resumption of Care (ROC) visits in patients’ homes
• Perform comprehensive assessments and develop individualized plans of care
• Accurately complete OASIS documentation within required timeframes
• Educate patients and caregivers on disease processes, medications, and care plans
• Coordinate care with physicians, therapists, and interdisciplinary team members
• Ensure compliance with state, federal, and agency regulations
