About The Position

Serves as the primary contact supporting individuals with Developmental Disabilities, Mental Health challenges for the START Program (Systematic, Therapeutic, Assessment, Resource, and Treatment) to provide community based intervention and prevention services to individuals with developmental disabilities and behavioral health care needs. The program relies on integrated care and treatment with collaboration among all team members. The services in the START Program include collaborative contacts, after hour contacts, emergency team meetings, planned respite and emergency respite.

Requirements

  • Bachelor’s degree in Social Work, Social Services, Human Services or Special Education required; Master’s degree a plus
  • Minimum 2 years experience working with persons with intellectual and/or developmental disabilities required; 1-year experience if paired with Master’s degree
  • Must meet requirements for both Qualified Developmental Disability Professional and Qualified Mental Health Professional.
  • Must have direct support experience supporting individuals with mental health and developmental disability diagnosis.
  • Must have experience with crisis management and crisis counseling.
  • Valid driver’s license
  • CPR, first aid, NCI certification and others that may be specified and/or required by future company regulations.
  • Must maintain proficiency in company sponsored training and certifications and privileging if required.
  • Ability to Travel
  • Complete criminal background
  • Name checked in the registries. (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities)
  • Drug testing
  • Education verification and other credentialing based on position requirements.
  • Proof of employment history or references (if required)
  • Positions that require driving Proof of driver's license, driver's insurance, and vehicle, IF required for providing transportation for individuals.

  • Master’s degree a plus
  • 1-year experience if paired with Master’s degree

Responsibilities

  • Develops and revises the cross systems crisis plans in a timely fashion.
  • Monitors the implementation of the plan.
  • Performs Case Management functions of linking and arranging for services and referrals.
  • Provides families and stake holders with individual-specific training and training in the knowledge, skills, and abilities required by the population and age to be served.
  • Provides consultation to the system on the clinical needs of the individual's care.
