About The Position

This position focuses on health care for individuals, families, groups, communities and populations which occurs in primary and specialty care outpatient clinics. Addresses in partnership and collaboration with other health care professionals the patient's wellness, acute illness, chronic disease, disability, and end-of-life needs. Responsible for patient advocacy, coordination of nursing and other health services, implementation of nursing services and continuity of care. Provides patient education when needed.

Requirements

• Graduate of an accredited school of nursing required.
• Current unrestricted Wisconsin RN license required.
• At least one year medical/surgical nursing preferred.
• CPR certification required.
• Excellent communication and organizational skills required.

Responsibilities

• Focus on health care for individuals, families, groups, communities, and populations in outpatient clinics.
• Collaborate with other health care professionals to address patient wellness, acute illness, chronic disease, disability, and end-of-life needs.
• Advocate for patients and coordinate nursing and other health services.
• Implement nursing services and ensure continuity of care.
• Provide patient education as needed.
