About The Position

The Psychologist functions as the primary mental health provider of the HBPC Interdisciplinary Treatment Team (IDTT) that provides comprehensive, interdisciplinary primary care services in the homes of Veterans with complex, chronic, and disabling diseases. In addition to primary care interventions, HBPC provides palliative care, rehabilitation, disease management, and care coordination services. HBPC targets Veterans in advanced stages of chronic disease, particularly those at high risk for recurrent hospitalization or nursing home placement. Outcomes expected of HBPC include effective management of chronic disease and reduction of inpatient days and total cost of care. The incumbent functions as the primary mental health provider of the HBPC team, providing mental health prevention, assessment, treatment, management, and professional consultation services. The Psychologist serves as a core member of the CLC IDTT and is present during weekly and as needed IDTT meetings and is available for mental health consultation for Veterans residing on the CLC. The CLC provides a home-like environment for Veterans needing either short or long-term care and has Veterans of different ages and abilities needing skilled nursing facility level of support. Incumbent will provide psychological expertise for Veterans throughout the healthcare gamut-from Veterans with short-term needs with the goal to return home to Veterans who are receiving end-of-life hospice care. Incumbent is available to assist with dementia-related challenges, medical compliance and other mental health concerns that arise within the CLC. The incumbent embodies the Whole Health mindset and establishes treatment goals that promote maximal psychological wellness to allow Veterans to maintain their highest practicable level of overall function.

Requirements

  • Must meet specific individual eligibility requirements in accordance with VHA policy for the Education Debt Reduction Program (EDRP).
  • Must submit EDRP application within four months of appointment.

Responsibilities

  • Provides screening, assessment, diagnosis, and treatment of depressive and anxiety related disorders, as well as other Axis I conditions, with an emphasis on the application of time limited, evidenced-based approaches.
  • Assesses and provides treatments for subclinical symptoms of dysphoria and anxiety, adjustment difficulties, and excessive use or misuse of alcohol or other drugs.
  • Provides psychological prevention services to Veterans at significant risk for psychological problems.
