About The Position

The UW Medical Center - Northwest Pharmacy has an outstanding opportunity for an Staff Pharmacist. WORK SCHEDULE Per Diem Rotating/Variable Shifts POSITITON HIGHLIGHTS The Pharmacist is responsible for providing a safe, efficient, and well-controlled drug distribution system and pharmaceutical care system/structure in the hospital. This is accomplished by filling medication orders, consulting with physicians and healthcare team members regarding drug therapy, disseminating relevant drug information, monitoring drug therapy, providing direct patient care and education, and participating in in-service education and quality assurance programs. The Pharmacist is also responsible for directly supervising the work of non-pharmacist personnel within the department. The Pharmacist is a professional employee operating under the administrative direction of the Clinical Manager. The Pharmacist performs within the University of Washington Medical Center’s policies, state and federal laws, and guidelines set forth by The Joint Commission. Requirements

  • One year of experience in pharmacy practice or completion of an American Society of Health Systems Pharmacists accredited residency training program.
  • A valid Washington State license to practice pharmacy or eligibility to obtain license prior to employment.
  • UW Medicine is committed to being a drug-free workplace. We require a pre-employment drug screen and background check.

Nice To Haves

  • Completion of an ASHP-accredited residency program and/or fellowship (or equivalent experience) is preferred.
  • At least 3 years (or equivalent) of direct patient care experience in hospital setting is preferred

Responsibilities

  • Engage in accurate medication preparation and dispensing: Review medication orders for accuracy and evaluate appropriateness of therapy.
  • Provide patient profile review, assess patient data (such as demographics, chronic medical conditions, allergies, lab values, incompatibilities, etc.), and interpret data relative to patient’s medication needs.
  • Provide patient profile review for drug-drug interactions, duplication of therapy, appropriate regimens, etc.
  • Accurately prepare and dispense medications
  • Proper charging practices and documentation for billing compliance
  • Document and maintain appropriate notes and records of dispensed medications and provide any other necessary and required documentation, according to established procedures.
  • Consistently follow hospital and department policies and procedures.
  • Engage in appropriate procurement of drugs to meet patient needs
  • Provide oversight and supervision to pharmacy residents, interns, externs, students, and pharmacy technicians as required by law
  • Provide effective, quality pharmaceutical care through direct patient care and collaboration with patient care teams: Engage in all aspects of therapeutic selection, implementation, monitoring, modification, and dispensing of drug therapy.
  • In cooperation with other pharmacists and pharmacy technicians, act as a resource and provide answers to drug information questions for patients, family, pharmacists, nurses, medical staff, medical students, pharmacy residents, pharmacy interns, and other hospital personnel
  • Promote appropriate, safe, and effective medication use through adherence to the UW Medicine formulary and therapeutic interchanges as approved by the hospital’s Pharmacy & Therapeutics Committee.
  • Medication control and security, including but not limited to: Inspect and assist nursing units for maintaining compliance with specified medication standards.
  • Ensure secure and effective storage and dispensing of medications
