About The Position

We are seeking a highly skilled and dedicated Clinical Staff Pharmacist to join our interdisciplinary team at La Paz Regional Hospital. The Clinical Staff Pharmacist plays a pivotal role in optimizing medication therapy, providing pharmaceutical care, and ensuring patient safety through comprehensive clinical pharmacy services. Hours of Operation: Monday – Friday: 06:30 – 15:00. Weekends and Holidays: 08:00 to 12:00

Requirements

  • Doctor of Pharmacy (Pharm.D.) degree from an accredited pharmacy program.
  • Licensed as a pharmacist in the state of Arizona, in good standing.
  • Strong clinical pharmacy knowledge, including pharmacokinetics and pharmacotherapy.
  • Excellent communication and collaboration skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Proficiency in pharmacy software and electronic health records (EHR) systems.
  • Clinical pharmacy experience preferred.

Nice To Haves

  • Board Certification in Pharmacotherapy (BCPS) or other relevant clinical certification preferred

Responsibilities

  • Medication Review: Conduct thorough medication reviews to identify potential drug-related problems, interactions, and contraindications.
  • Patient Consultations: Collaborate with healthcare providers to provide medication therapy management, medication reconciliation, and patient counseling.
  • Clinical Interventions: Participate actively in interdisciplinary rounds and provide real-time drug information and recommendations to healthcare teams.
  • Protocol Development: Assist in developing and implementing clinical pharmacy protocols and guidelines to enhance patient care.
  • Medication Management: Manage complex medication regimens, including dose adjustments and therapeutic monitoring.
  • Pharmacokinetics: Monitor and adjust medication dosages based on pharmacokinetic principles when required.
  • Antimicrobial Stewardship: Participate in antimicrobial stewardship programs to optimize antibiotic use and combat antibiotic resistance.
  • Medication Safety: Promote medication safety by conducting root cause analysis for medication errors and implementing strategies for prevention.
  • Patient Education: Provide patient education on proper medication usage, side effects, and potential drug interactions.
  • Quality Assurance: Contribute to continuous quality improvement efforts within the pharmacy department and healthcare facility.
  • Documentation: Maintain accurate and organized patient records, clinical interventions, and drug utilization evaluations.
  • Regulatory Compliance: Ensure compliance with all federal and state pharmacy regulations and standards, including HIPAA, USP, DEA, and Board of Pharmacy.
  • Preceptorship: May serve as a preceptor for pharmacy residents, interns, or students.
