About The Position
Northwest Community Health Center (NWCHC) is looking for a team-oriented full time Staff Pharmacist to join the organization in Libby Montana. The selected candidate will serve a critical role in the general operation of NWCHC's in-house pharmacy in compliance with Montana State Board of Pharmacy, properly dispensing medication ensuring medications are correctly entered, supplied, and labeled. Additional operation duties will include adherence to patient safety protocols, pharmacy professional practice, regulatory requirements, quality assurance, customer service, inventory management, loss prevention, and workflow management. In addition, as a member of the integrated primary care team, the Pharmacist will be working with a multidisciplinary primary care team to effectively perform Collaborative Medication Management via protocol partnering with other clinicians to help patients receive the support necessary to impact their overall health. As part of the healthcare team, this position is responsible to resolve clinical questions regarding patients' medication regimens, and answer clinical questions from provider staff. He or she will perform medication therapy management consultations including medication reconciliations, screening for adverse effects and drug interactions, providing disease/medication education, and exploring opportunities to simplify and/or decrease the cost of the medication regimen.
