About The Position

Requirements

  • Bachelor's Degree in Pharmacy; required
  • Doctorate of Pharmacy (PharmD); preferred
  • Graduate of an accredited school of Pharmacy; required
  • Licensed as a Pharmacist in the state based on facility location by the State Board of Pharmacy; required
  • Minimum of 3 years experience in comparable health system pharmacy; required
  • American Society of Health Systems Pharmacy Residency (ASHP) or 3 years clinical practice experience; required
  • Excellent written and verbal communication skills; required
  • Demonstrated proficiency in multiple clinical or operational areas; required
  • Ability to work independently and improve patient quality outcome measures.

Responsibilities

  • Assists in coordinating and providing care to patients across the continuum of care.
  • Serves as a resource person to support pharmacy personnel and other healthcare professionals.
  • Assists in designing and coordinating clinical pharmacy services.
  • Monitors and manages drug therapy in designated patient populations.
  • Develops care plans and guidelines including drugs, devices, and delivery modalities.
  • Provides pharmacotherapy education to patients and healthcare professionals.
  • Participates in educational programs focusing on disease prevention and wellness promotion.
  • Maintains continuous education hours as required.
  • Assists in the prevention, detection, and resolution of medicine abuse.
  • Verifies patient health insurance benefits and pre-certifications for Third Party Payers.
  • Maintains service activities and outcomes reports for designated practice areas.
  • Coordinates and participates in pharmacology research and development.
