This position is for the following schedule: Tuesday-Saturday 10am-6pm Cantata Adult Life Services, formerly known as The British Home, is a nonprofit, mission-driven community with over 100 years of experience, offering a full continuum of care on a beautiful 10-acre campus in Brookfield, Illinois. General Summary The Staff Nurse is responsible for daily clinical operations on the unit, ensuring high-quality resident care, regulatory compliance, and effective team coordination. This role serves as a guide, mentor, and resource to residents and co-workers, while supporting interdisciplinary collaboration and safe care transitions. Requirements

  • Active RN or LPN license in the state of Illinois
  • Clinical experience in skilled nursing or acute care (preferred)
  • Strong knowledge of long-term care regulations (IDPH/CMS)
  • Excellent communication, organization, and leadership skills
  • Ability and willingness to work harmoniously within a team environment departmentally.
  • Ability to communicate effectively both verbally and in written form with a diverse group of people.
  • Must possess the ability to deal tactfully with personnel, clients/patients, family members, visitors, government agencies/personnel and the general public.
  • Must possess the ability to make independent decisions when circumstances information includes, but shall not be limited to, information concerning Cantata/British Home former and current clients/clients, medical and health information and Cantata/British Home information systems including but not limited to the Internet and E-mail.
  • Must meet the general health requirements set forth by the policies of this facility.
  • Must be able to frequently push, pull, move 20-30 lbs., and occasionally lift a minimum of 50 pounds.
  • Sits, stands, bends, lifts, walks and climbs stairs, as needed to perform tasks/duties.
  • May be scheduled to work weekends and holidays based on operational needs.
  • Schedules are subject to change based on operational need.
  • May be necessary to assist in the evacuation of clients during emergency situations.
  • Aptitude for detail work.
  • Must be able to read, write, speak, and understand the English language.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility.
  • Must stay on unit until relieved by oncoming staff. If oncoming staff is late or missing inform the scheduler/DON.

Responsibilities

  • Supports the Mission, Values, and Vision of Cantata Adult Life Services and the facility.
  • Supports, cooperates with specific procedures and programs for:
  • Safety, including universal precautions and safe work practices, established fire/safety/disaster plans, risk management, and security, report and/or correct unsafe working conditions, equipment repair and maintenance needs.
  • Confidentiality of all data, including client, employee-partner and operations data.
  • Quality Assurance and compliance with all regulatory requirements.
  • Compliance with current law and policy to provide a work environment free from sexual harassment and all illegal and discriminatory behavior.
  • Provides quality nursing care to clients in an environment which promotes their rights, dignity, freedom of choice, and their individuality as illustrated by the following:
  • Direct and supervise CNA staff (CNAs) during assigned shift
  • Ensure delivery of safe, high-quality, person-centered care
  • Conduct rounds to monitor resident status, safety, and care delivery
  • Serve as primary clinical decision-maker in the absence of management
  • Respond to changes in resident condition and initiate appropriate interventions
  • Provide real-time coaching, guidance, and conflict resolution
  • Ensure staff adherence to policies, procedures, and scope of practice
  • Support orientation and training of new staff
  • Promote teamwork and a positive work environment
  • As required/needed perform routine nursing tasks, to include but not limited to, administration of medications and prescribed treatments, according to the care plan.
  • Coordinate shift-to-shift communication of significant/required nursing information utilizing procedures and forms established by The British Home for this purpose.
  • Assure that sufficient supplies and equipment are available for provision of patient care.
  • Make written and oral reports as required/needed to physicians, pharmacists, therapists, and other licensed providers of service to enable proper follow-through of orders, coordination of care services, communication of nursing observations suggesting the need for further medical evaluation as well as notification to resident/patients responsible party.
  • Provides individualized attention, which encourages each client’s ability to maintain or attain the highest practical physical, mental, and psychosocial well-being.
  • Knowledgeable of the individualized care plan for clients and provides support to the client according to the care plan.
  • Attends to the individual needs of clients which may include assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation and range of motion, applying creams/ointments, administering medications or other needs in keeping with the individuals’ care requirements.
  • Support restorative care and maintain resident comfort, privacy, dignity, and independence
  • Interacts with clients in a warm, compassionate manner that fosters a caring, homelike environment.
  • Fully understands all aspects of client rights, including the right to be free of restraints and free of abuse. Responsible for promptly reporting to the charge nurse or administrative staff incidents or evidence of client abuse or violation of clients’ rights.
  • Answers clients’ call lights promptly and courteously.
  • Provides care that maintains each client’s skin integrity to prevent pressure ulcers, skin tears, and other damage by changing incontinent clients, turning, repositioning immobile clients, and applying moisturizers to fragile skin, etc.
  • Observe clients for changes in condition or behavior and promptly reports these changes to appropriate licensed nursing personnel.
  • Monitor residents for safety risks (falls, skin breakdown, changes in behavior or cognition)
  • Communicate unit updates, concerns, and priorities to the NP and DON
  • Ensure accurate, timely, and complete documentation in the EMR (e.g., Matrix-Care)
  • Completion of required assessments (admission, discharge, pain, etc.)
  • Ensure compliance with IDPH, CMS, and facility policies
  • Prepare for and support regulatory surveys
  • Oversee admissions, transfers, and discharges for clinical readiness
  • Verify orders, medications, and care plans are implemented correctly
  • Support safe discharge planning in collaboration with the care team
  • Identify and escalate barriers to discharge
  • Help Agency staff understand Matrix and charting requirements.
  • Assists in maintaining a safe, neat, and clean environment; reports environmental deficiencies to the EVS such as lighting or equipment problems.
  • Performs all job responsibilities in accordance with prescribed safety and infection control procedures including thorough hand washing, PPE, and proper disposal of soiled materials.
  • Practices careful, efficient, and no-wasteful use of supplies and linen and follows established charge procedures for client charge items.
  • Communicates effectively and tactfully with clients, families, visitors, peers, and supervisors, including in sensitive or emotional situations.
  • Works collaboratively with team members, using sound judgment to plan and complete duties with minimal supervision.
  • Follows up appropriately on concerns, complaints, and care-related issues with supervisors, co-workers, and clients as needed.
  • Promotes positive relationships and a professional image with clients, families, and guests.
  • Assists with orienting clients and families upon admission or transfer to a new unit.
  • Ambulates throughout the unit to complete rounds and safely transport clients within the facility.
  • Completes required in-service training and maintains compliance with attendance, uniform, dress code, and personal hygiene standards.
  • Assists the supervisor with administrative tasks and special projects to support department operations, as assigned.
  • Performs other duties as assigned.
