The University of Iowa Community Clinics (UICC) is seeking a patient centered 100% Staff Nurse to work at our Scott Blvd Clinics . This position will mainly support our Scott Blvd Family Medicine, Pediatric, and Psychiatry Clinics but could float to other patient specialties/clinic locations. This position will support the clinic in providing phone triage support, laboratory support, helping with prior authorizations, as well as assist with nurse visits. Our Family Medicine staff can see everyone in the family, in every generation, from birth, for life. We see patients for pregnancy care, baby wellness, vaccinations, school and athletic physicals, acute care visits, annual physicals, regular check-ups, mental health, on-site laboratory services and more. And when anyone in the family needs specialty care, our family medicine provider connects our patients with the UI Health Care specialist needed. UI Health Care specialists will consult the same medical record used by our family medicine provider so our patient's specialty care is streamlined. Assesses patient health care preferences and expectations. Collaboratively coordinates the continuum of care across settings. Develops patient care plan based on patient condition, age, and psychological, educational, and socioeconomic conditions that prescribes interventions to attain expected outcomes. Implements patient care plan according to patient priorities, which includes interventions identified through nursing diagnoses. Documents/reports interventions and patient/family response. Collaborates and integrates contributions of patients and families into multidisciplinary patient care plan. Develops, coordinates and implements transfer/discharge planning throughout episode of care. Implements nursing interventions according to department, division, and/or unit policies, procedures, and protocols to maintain safe environment for patients, visitors and staff. Follows universal precautions.
