The University of Iowa Community Clinics (UICC) is seeking a patient centered 50 or 100 % Staff Nurse to work within our new Endocrinology Clinic located in Bettendorf, IA . Our clinics specializes in the expert care for diabetes and other conditions that affect your body’s glands, hormones, and metabolism. We provide personalized diabetes treatment plans and follow-up care. UI Diabetes Care is a leader in the treatment of people with diabetes and has developed innovative programs designed to help prevent and control diabetes. We use a patient-centered care approach—a partnership between you and your team of physicians, nurses and dietitians—to develop a diabetes treatment plan that meets your needs. This position will mainly support Endocrinology but could also support other service lines/clinics based on clinical needs/hours. Our Staff Nurses develop relationships and get to know our patients and monitor each family member’s health care history and deliver personalized, compassionate care every time to each patient. Applies nursing theory to the assessment, diagnosis, treatment, and evaluation of patient responses to health and illness. Assesses patient health care preferences and expectations. Collaboratively coordinates the continuum of care across settings. Telephone tirage to support Bettendorf clinics; Dermatology, Family Medicine, Endocrinology, and Pediatric Outreach. Develops patient care plan based on patient condition, age, and psychological, educational, and socioeconomic conditions that prescribes interventions to attain expected outcomes. Implements patient care plan according to patient priorities, which includes interventions identified through nursing diagnoses. Documents/reports interventions and patient/family response. Collaborates and integrates contributions of patients and families into multidisciplinary patient care plan. Develops, coordinates and implements transfer/discharge planning throughout episode of care. Implements nursing interventions according to department, division, and/or unit policies, procedures, and protocols to maintain safe environment for patients, visitors and staff. Follows universal precautions.
