Duties include but are not limited to triage; access;  care coordination; and nursing support of clinical sessions.

Requirements

  • Successful completion of a nursing program (RN), BSN preferred
  • Current MA RN License, current BLS (Basic Life Support) certification required
  • Experience in a community health setting preferred, ambulatory care or health insurance     environment experience preferred
  • Basic computer literacy, knowledge and experience with Microsoft Office 365 including Word, Excel, Outlook and Teams, knowledge of Electronic Medical Record (EMR), EPIC/OCHIN  preferred. 
  • Excellent customer service skills, excellent verbal and written communications skills
  • Ability to effectively work within both a team setting and independently
  • Ability to work effectively with economically, racially and linguistically diverse customers
  • Bilingual a plus, English and Vietnamese/Spanish/Portuguese or Haitian Creole

Responsibilities

  • Performs all nursing duties in accordance with Massachusetts Nursing Practice Act, while adhering to Harbor Health policies and procedures.
  • Assists providers and Care Team members in caring for patients, acts as liaison between the patient, the care team and/or provider.
  • Performs routine nursing care to patients, while utilizing age specific guidelines and standard precaution practices at all times.
  • Performs comprehensive assessment of patients health status and triage based on urgency of symptoms, in person and via telephone.
  • Actively demonstrates a working knowledge of nursing theory, techniques, principles, and practice to holistically care for the patient.
  • Provides patient education on disease prevention, medications and treatment plans and self-care techniques.
  • Provides health coaching and teaching for common conditions (i.e. asthma, hypertension, diabetes) through individual nurse visits, group visits and co-visits with other disciplines () as appropriate.
  • Helps patients stay up to date with clinical care guidelines, immunizations and health maintenance  screenings.
  • Administers medications and vaccinations to as ordered by provider or by following standing orders policy.
  • Assesses information about the patient’s physiological, psychological, sociological, and spiritual status. 
  • Assists in the orientation, evaluation, and training of new staff to the department.
  • Documents patient care activities, assessments, treatment plans and interventions in EHR, ensuring accurate and timely documentation of patient encounters.   
  • Communicates with staff, patients, and families demonstrating excellent written and verbal skills.
  • Consistently seeks educational tools to broaden knowledge base of various age groups, diseases, and best practices, maintains knowledge of all medications, reactions, documentation, and dosage requirements.
  • Conducts procedures according to policy while ensuring patient safety.
  • Certifies that emergency equipment in clinical area is appropriately maintained and in working order. 
  • Follows HHSI policy when disposing of biohazardous materials, labeling and handling all materials.
  • Participates in team, department, and clinic meetings to learn, support process improvement, and contribute to care plans as appropriate. 
  • Provides cross coverage as needed (Primary Care, Women’s Health and Pediatrics.)
