About The Position
The candidate will provide case management services to clients in our Healthworks/Medical Case Management for children at risk, High-Risk Infant Follow-up, and Better Birth Outcomes programs. Home visits are required. The schedule is Monday through Friday, 8:30 a.m.- 4:30 p.m.
