Requirements

  • Bachelor's degree in job related field plus one (1) year of responsible experience

Nice To Haves

  • Intermediate skill level in using Microsoft Office programs.
  • Bilingual(English/Spanish)
  • Background in running/overseeing clinic operations, building programs in clinics and working with government programs.

Responsibilities

  • Plan, coordinate, and implement GUIDE program initiatives and events to engage rural healthcare professionals and promote dementia care, awareness and services.
  • Coordinating assessments within GUIDE program of patients.
  • Develop and manage relationships with healthcare professionals, organizations, community groups, and government entities to support and provide GUIDE program initiatives.
  • Assist in developing, managing/maintaining a database of contacts and resources for GUIDE program.
  • Perform other duties as assigned in order to accomplish the mission/goals of GUIDE program and the GIA.
  • Prepare and submit progress reports as requested/required to director of Memory Clinic.
  • Represent GUIDE program and the GIA at community events and meetings as needed.
