About The Position

We’re looking for a Staff Backend Engineer to help build the next generation of Mochi’s pharmacy software platform. This is a rare opportunity to work at the intersection of telehealth and pharmacy operations, where the backend systems you design will not just support pharmacies, they will set the standard for how they operate. As a senior member of the engineering team, you will own the design and development of scalable, reliable backend systems that power critical pharmacy workflows. You will architect distributed systems, manage high-volume integrations, and ensure our infrastructure supports real-time interactions between patients, providers, and pharmacies. This role goes beyond writing code. You will be driving backend architecture, influencing technical direction, and collaborating across teams to ensure our platform remains performant and secure as we scale. Your work will directly impact how pharmacies deliver care and how patients experience healthcare.

Requirements

  • 7+ years of backend development experience, with deep expertise in Node.js (TypeScript/JavaScript) and modern backend frameworks.
  • Proven experience architecting and deploying distributed systems in production at scale.
  • Strong knowledge of cloud infrastructure (AWS preferred), container orchestration (Kubernetes), and CI/CD pipelines.
  • Skilled in building products end to end, from design to deployment, and improving maintainability and performance in large codebases.
  • Strong collaborator with excellent written and verbal communication skills, able to work cross-functionally with both technical and non-technical teams.
  • Thrive in a startup environment, adaptable, resourceful, and driven to solve complex problems with real-world impact.
  • Excited to work in-person with our team in San Francisco.

Nice To Haves

  • Experience with healthcare software, pharmacy systems, or regulated environments.
  • Familiarity with real-time messaging, inventory and fulfillment systems, or enterprise SaaS platforms.

Responsibilities

  • Lead the development and optimization of backend services, APIs, and data pipelines that power pharmacy operations at scale.
  • Architect and manage systems supporting high webhook traffic, third-party integrations, real-time scheduling, provider and patient management, and compliance workflows.
  • Drive best practices in performance, security, observability, and scalability as we rapidly expand pharmacy operations.
  • Collaborate closely with Product, Frontend, and Pharmacy Operations teams to build seamless workflows that meet complex healthcare needs.
  • Own key architectural decisions, mentor engineers, and set backend engineering standards across the company.
  • Ensure system reliability and resilience in distributed environments, with a focus on patient safety and operational efficiency.
  • Influence engineering culture, fostering technical ownership and innovation within the team.
