About The Position

Community Programs is a program office within Health Services Administration, a division under the Los Angeles County Department of Health Services (DHS). Community Program’s mission is to improve health equity and providing supportive services to vulnerable residents, including those experiencing homelessness, justice-involved (JI) individuals, and high-need patients. CalAIM a State Medi-Cal waiver program. The CalAIM team coordinates the operational implementation of housing community support benefits within the Office of Diversion and Re-entry (ODR). CalAIM team leads the organization of the ODR housing programs to ensure State and payer compliance and streamlines workflows that maximize revenue and quality control for the claims data. The team also plays a critical role in interpreting, influencing, and implementing existing and new CalAIM policies at the State and local levels. The CalAIM team’s leadership and staff will actively work across different stakeholder groups including State and County policymakers, County leadership, and advocacy groups to support operations and sustain funding. This Staff Analyst position will function as a Justice Involved Project Manager and will play an essential role as part of the CalAIM Housing and Reentry Initiatives team within Community Programs. This role will coordinate the tracking and implementation of Justice Involved services within LA County across all County teams including Correctional Health Services, Department of Mental Health & the Department of Public Social Services within LA County.

Requirements

  • Five (5) year analyzing and making recommendations that address organizational management issues such as planning, program development, project management, budget development, and program evaluation of health and human services for high-risk populations (e.g. homeless, justice-involved, mentally ill, substance use disorder or medically complex).
  • Successful clearing through the Live Scan process within the County of Los Angeles.

Nice To Haves

  • A Bachelor's degree or higher from an accredited college or university in Public Health, Public Administration, Business Administration, Healthcare Administration, Healthcare Management, Biostatistics, Epidemiology, Social Work, Community Development, Criminal Justice or a related field
  • MPHs (Master of Public Health)

Responsibilities

  • Coordinate implementation of the CalAIM Justice Involved services across County teams, i.e., CHS, DMH & DPSS.
  • Monitors workplan workflows data, legal, IT, and program teams to ensure successful implementation of CalAIM JI pre-release and Specialty Mental Health Services.
  • Manages cross-functional dependencies and works towards reducing delays tied to communication breakdown of vital and timely processes.
  • Convenes and facilitates discussions with stakeholders to design and implement tools and reports
  • Manages the design, development, implementation and maintenance of reports, dashboards and analysis tools
  • Provides Program Management to ensure CalAIM work is prioritized appropriately and that all deadlines are met.
  • Provides regular reporting within specified deadlines
  • Regularly performs special projects and assignments, such as ad-hoc department reporting and program analyses
  • Rigorously documents processes and procedures used for all reporting and analyses
  • Develops ongoing and special project reports and assignments including ad-hoc department reporting and program analyses.
  • Manages the design, development, refinement, implementation and maintenance of reports, dashboards, and analysis tools used for the program management practices within the CalAIM Housing and Reentry Initiatives Program.
  • Provides detailed programmatic analysis and makes recommendations for ongoing operations and special projects as a CalAIM subject matter expert.
  • Helps refine program management practices through the creation of new reporting and analytical tools
  • Helps to design, build and maintain analytical support tools to be used within the Department of Health Services, by housing and service providers, and by other external stakeholders
  • Develops new programmatic processes and/or recommended modifications to existing programs based on needs analysis in accordance with local, State, and/or federal requirements.
  • Support efforts to plan and implement pre- and post-release CalAIM Enhanced Care Management (ECM) Programs for justice-involved individuals and coordinate Correctional Health Services clinical care transition efforts with community-based re-entry providers.
  • Assist in conducting population level assessments of the justice involved population to guide the scale and scope of CalAIM services to address needs and support strengths in areas including health, mental health, substance use, housing, family/social support, education, employment, and benefits eligibility.
  • Work with DHS teams to support the development and implementation of a pre-release model of care under CalAIM that includes but is not limited to: eligible patient identification, ECM application submission, benefit applications, assignment to community based providers, in-reach of community based providers, care plan development, and community care coordination.
  • Develop workflows to integrate Correctional Health Services (CHS) clinical services with ECM-oriented transition and diversion efforts to support safe transitions to the community following incarceration.
  • Coordinates and participates in meetings with departmental, public, private, and non-profit groups and organizations as a County representative to discuss policy issues impacting the CalAIM Housing and Reentry Initiatives Program.
  • Conducts and facilitates discussions including programmatic/service negotiations with CalAIM stakeholders including County Department officials, members of community- based organizations, as well as the public as a representative of LA County Health Services and Community Programs.
  • Serves as a team lead role within the CalAIM Housing and Reentry Initiatives Program team designing and implementing new programmatic workflows.
  • Researches and analyzes protocols, best practices, policy issues, pending legislation, regulations, potential funding sources, and technical research, and makes recommendations to management regarding impact on CalAIM services.
