About The Position

As a Senior Patient Care Integrator, you will be at the forefront of ensuring that every patient receives comprehensive, compassionate, and well-coordinated care. In this role, you will act as the bridge between patients and healthcare providers, overseeing the seamless integration of care plans and services. Requirements

  • Associates’ degree in nursing, social work, or a related healthcare field with a strong emphasis on patient-centered care is required.
  • Minimum of five years of experience in a healthcare setting, with proven experience in patient care coordination and leadership.
  • Demonstrated ability to manage and integrate complex care plans, ensuring patients receive seamless and high-quality care.
  • Self-directed with strong organizational skills and a focus on attention to detail to ensure that every patient’s care needs are fully addressed.
  • Excellent communication skills, both written and verbal, with the ability to interact compassionately with patients, families, and healthcare professionals.
  • Creative problem-solving skills, with a commitment to improving patient care delivery and ensuring patients have a positive and supportive experience.
  • High level of integrity and commitment to maintaining confidentiality in managing patient health information.
  • Proficiency in MS Office, Excel, and electronic health records systems.
  • Ability to perform tasks requiring fine motor skills, including the use of computers and office or medical equipment to ensure effective patient care management.
  • Strong visual acuity for reviewing patient records, clinical data, and other medical information.
  • Able to work at a computer for extended periods, with breaks as needed, while maintaining focus on care coordination.
  • Capable of lifting up to 25 pounds as needed.
  • Able to sit for extended periods while managing patient care responsibilities.
  • Ability to travel overnight for training and/or meetings.
  • Must hold a valid driver’s license in good standing.

Responsibilities

  • Integrate and oversee all aspects of patient care requests, ensuring timely, compassionate responses that prioritize patient needs and well-being.
  • Serve as the central point of contact for patients, guiding them through their healthcare journey and ensuring seamless coordination with providers, specialists, and the care team.
  • Manage and maintain accurate and up-to-date Electronic Medical Records (EMRs), ensuring that patient care information is readily accessible for all care providers.
  • Coordinate patient medical testing, procedures, and follow-up care, ensuring patients are fully informed and confident in their care plans at all times.
  • Develop comprehensive and personalized care summaries and medical briefs for each patient, helping them understand their treatment path and making sure they feel supported.
  • Oversee the customization and distribution of Personal Medical Kits (PMKs) tailored to the individual medical needs of each patient, ensuring they are prepared for any health situation.
  • Engage in the patient-centered on-call rotation, ensuring 24/7 support for patients within 90 days of employment.
  • Actively contribute to other patient care initiatives and responsibilities, always with a focus on providing the highest level of patient care integration.
  • Other duties as assigned.
  • Ensure compliance with all state and federal healthcare regulations, including HIPAA, while maintaining the highest standards of patient confidentiality.
  • Promote a positive and patient-first approach in all interactions, ensuring a compassionate and empathetic attitude is consistently demonstrated when working with patients and colleagues.
  • Support the care team in managing unexpected or complex situations, ensuring patients receive excellent care through calm and effective problem-solving.
