CommWell Health is seeking a compassionate, organized, and clinically strong Senior Director of Nursing who thrives in a mission-driven environment and is passionate about coordinating care for patients living with multiple chronic conditions. In this role, you’ll work as part of a multidisciplinary team focused on population management, helping patients improve outcomes through education, care planning, and strong transitions of care support. Requirements

  • Active Nursing license, Associate degree in nursing at minimum
  • CPR/BLS - Active
  • HS Diploma or GED
  • Valid NC driver’s license + clean driving record (approved for driving privileges)

Responsibilities

  • Oversee assessment of daily nursing operations and integration of services with dental, behavioral health, specialty referrals, as well as, other internal and external services needed for disease and case management.
  • Process improvements and development in clinical operations utilizing rapid cycle improvement and reengineering, as needed.
  • Work closely with education and quality data management department to ensure foundational support is provided.
  • Timely and ongoing professional development, education, and competency evaluation of staff that provide any degree of patient interaction.
  • Drive changes in a supportive, empowering and educational manner to achieve objective and desired outcomes.
  • Ensure that all patients receive necessary quality care; working closely with patient experience as well.
  • Ensure JCAHO and HRSA readiness, compliance, and directs and oversees attainment of goals and objectives.
  • Develops, revises and reviews nursing policies and procedures as necessary.
  • Oversee and coordinate clinical nursing programs.
  • Work with COO/CNO to set objectives and long-term goals for nursing department.
  • Work in collaboration with Quality to identify and implement changes necessary to improve quality measures.
  • Oversee and manage organization wide infection control program.
  • Develop, review and revise all infection control policies and procedures.
