The Healthy Transitions Clinician will provide clinic and community-based services to youth and young adults ages 16-25 with significant mental health concerns. Healthy Transitions is designed to assist clients with meeting personal objectives for improving mental health, obtaining employment, reaching educational goals, and making successful transition to adulthood. The clinician will apply clinical skills to provide behavioral health services and supports to meet the needs of clients. The clinician will be the primary contact person for clients and families. The clinician’s primary goals are to build a positive relationship with participants and assist them in developing their abilities for illness-management. The clinician will provide support, education, consultation, and basic services to the participants and their families.
