About The Position

Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. For well over a decade, Concordia Visiting Nurses has provided high-quality skilled nursing, palliative care, and a variety of therapy services to our patients. We are looking for a compassionate and faith-driven Spiritual Care Coordinator to provide emotional and spiritual support to patients and families during some of life’s most meaningful and vulnerable moments. Requirements

Experience in spiritual care, pastoral care, ministry, chaplaincy, hospice, home health, or a related setting
A compassionate and patient-centered approach
The ability to provide support to patients and families from diverse backgrounds and belief systems
Strong communication and documentation skills
The ability to work independently while collaborating with an interdisciplinary team
A valid driver’s license and reliable transportation for travel within the service area

Responsibilities

Provide spiritual support and counseling to patients, families, and caregivers
Assess spiritual needs and help incorporate them into the patient’s overall plan of care
Offer comfort, guidance, and emotional support during illness, decline, grief, and end-of-life care
Collaborate with nurses, social workers, aides, and other members of the interdisciplinary team
Participate in care planning and team meetings
Support patients and families in accordance with their personal beliefs, values, and preferences
Document visits, interventions, and patient/family needs in a timely manner
Serve as a spiritual support resource for both patients and staff as needed
