About The Position

The coordinator of Spiritual Care is directly responsible to Patient Care Coordinators for providing spiritual counsel and emotional support to the active and bereaved Hospice patient/family units and to act as liaison between community clergy and Hospice. The Coordinator of Spiritual Care is committed to the philosophy, goals and objective of the Hospice Concept.

Requirements

  • Master of Divinity or theological equivalent from an accredited school
  • Current, valid ordination and/or endorsement in a major faith group
  • Certification or certification eligible (Chaplaincy or Pastoral Care) by a national professional credentialing organization
  • Respect for and familiarity with a wide range of religious perspectives and personal, pastoral presence
  • Two or more years' experience in pastoral ministry
  • Ability to drive across several counties and participation in in-call coverage
  • A valid driver's license is required (if you do not have a valid Ohio driver's license you must obtain one within one year of your residency in the state) and you must also meet BVHS's company fleet policy requirements
  • Positive service-oriented interpersonal and communication skills required.
  • Individual must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served on his/her assigned unit/department. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient status. Must be able to interpret the appropriate information needed to identify each patient's requirements relative to their age-specific needs and to provide the care needed as described in the area's policies and procedures
  • This position is classified “at risk” for possible occupational exposure to blood borne pathogens (HBV, HIV, etc.)

Responsibilities

  • Demonstrate knowledge and skills necessary for age appropriate pastoral care of Hospice patients/families through visitation, spiritual assessments, and interdisciplinary team care planning. (Knowledge should include pastoral care, counseling, psychological and theological integration in meeting patient/family needs.)
  • Develop effective relationships and communication with other team disciplines and within the community to ensure exceptional patient/family care. Identify spiritual, emotional, and other needs and make appropriate plan of care or referrals.
  • Assist patients/families and colleagues in coping with grief and loss, both by professional training and through modeled behavior.
  • Maintain assigned patient caseload within scheduled hours while minimizing overtime. Ability to work independently while communicating effectively with supervisor and team members for planning caseload and/or other demands of time, including team meetings.
  • Accurate documentation of visits and care according to guidelines of Bridge Home Health and Hospice. Compliance with other organizational policies, procedures, and practices to ensure quality job performance and Medicare/Medicaid compliance
  • Assist in planning and conducting memorial services/funerals as needed by patients/families.
  • Displays Service Excellence as evidenced by practicing the mission, vision and values of the organization to promote patient satisfaction.
